The California Consumer Privacy Act of 2018 (“CCPA”) affords certain rights to the residents of the State of California. The following disclosures apply only to the Personal Information of California residents that is subject to the CCPA.

Verification Process and Required Information
We may need to request additional information from you to verify your identity or understand the scope of your request, although you will not be required to create an account with Us to submit a request or have it fulfilled. We will require policyholders and claimants to provide, at a minimum, their name, address, email address, and claim or policy number.
Authorized Agent
You may designate an authorized agent to make a CCPA request on your behalf. If so, you must provide the authorized agent written permission to do so, We may require that you verify your identity directly with Us and We may require other proof of the agent’s authority to act on your behalf.
